Principles of bayesian therapeutic drug monitoring (TDM) are used for computerized individualizing drug dosage regimens. TDM Software have been created to help physicians but have to be evaluated use in practice. The aim of this study is to evaluate three softwares for individualizing hospital drug regimens with digoxin, phenytoin and ganciclovir. We compared expected concentrations we got using these softwares with measured concentrations. We show that the three evaluated softwares are acceptable for the three choosen drugs but their functional limits (i.e. only one of them can work for drugs with non-linear pharmacokinetics like phenytoin) have to beknown for a reasonable use. The three evaluated programs are very different. The choice between them for use in a clinical pharmacology service is function of material and scientific factors. This work show the evident interest of computerized TDM for drugs with large inter- and intra-individual differences but quality assurance of TDM is important in order to optimize drug dosage regimens.
