Cell cycle regulatory factors of the esophageal squamous cell carcinoma as indicators of its prognostic relevance

In a search for markers that can assist surgeons to determine the indications of less invasive operations for esophageal squamous cell carcinoma, the expression of the cell cycle regulators, p21, p27, p16, cyclinD1 and proliferating cell nuclear antigen (PCNA), was investigated by immunohistostaining. The results were compared with the clinicopathological factors including the prognosis. p21 and cyclinD1 were found to be correlated with lymph node metastasis, clinicopathological stage and survival rate. p16 was correlated with lymph node metastasis and survival rate. PCNA-PI was high in the p21 positive group but low in the cyclinD1 positive group. By performing preoperative biopsies to evaluate the expression of p21, p16 and cyclinD1, it is considered that the individual tumor characteristics can be assessed, and this information can help surgeons to decide the operative procedure of choice. Less invasive surgery is thought to be indicated in patients without lymph node metastasis. Patients with a high p21 or p16 positive ratio and a low cyclinD1 positive ratio, are unlikely to have lymph node metastasis.
