Evaluation of hypothalamic-pituitary function in acne patients using the LH-RH and TRH stimulation test

To investigate the role of the hypothalamic-pituitary function in acne patients, LH-RH and TRH stimulation tests were performed in 7 male patients with nodular-cystic acne and 26 female patients with postadolescent acne. Blood samples were taken prior to, and 15 and 30 min after the TRH and LH-RH stimulation. In the female patients, TRH, LH and FSH values, both basal and stimulated, did not show any significant difference from the control. However, significant elevation of PRL levels 15 min and 30 min after stimulation was noted in the female patients, compared with the control group. The result suggests mild hypophyseal hyperprolactinemia. In the male patients, PRL, TSH and FSH, both basal and stimulated, did not differ significantly from the controls. However, LH levels after the stimulation were significantly higher in the patients. These results suggest that abnormal hypophyseal feedback mechanisms might contribute to the development of acne lesions.
