Iron metabolism and diagnostic approach to iron deficiency in inflammatory disease

Anemia is a frequent finding among hospitalized patients. In case of inflammatory disease either chronic due to rheumatic disease or due to severe infection anemia of inflammatory origin, previously described as anemia of chronic disease (ACD), can be observed. Often, this type of anemia cannot be distinguished from iron deficiency anemia, and in some cases both reasons for anemia may coexist. The anemia of inflammatory disease is cured by treatment of the underlying disease, iron deficiency anemia must be corrected by iron supplementation. Since there is no single marker allowing to distinguish between both types of anemia the present review addresses the pathogenesis of anemia of inflammatory disease the recent literature on the diagnostic parameters being used to characterise iron metabolism in anemia of chronic inflammation.
