Antibacterial therapy of urogenital mycoplasma infections. Comparative study of wilprafen and doxycycline efficiency

This study was made to compare therapeutic efficiency of wilprafen (16-membered macrolid antibiotic) and doxycycline in patients with urogenital inflammation caused by mycoplasma infection. The randomized study involved 56 patients (17 men and 39 women, mean age 28 years) with urethritis and cervicitis associated with U. uralyticum or M. hominis infection. The infection was diagnosed by polymerase chain reaction (PCR) using epithelial scrapes from the cervical canal of the uterus and/or urethra. The patients were randomly allocated to two groups. Patients of group 1 (n=29) received wilprafen (500 mg twice daily for 12 days), those in group 2 (n=27) were given doxycycline (100 mg twice daily for 10 days). After a follow-up of 4 weeks, all patients were examined to identify changes in the clinical course of the disease and the presence of infection (repeat PCR). The therapy resulted in the improvement of clinical features in the majority of patients. They persisted in four of them receiving doxycycline and in one receiving wilprafen. Wilprafen therapy resulted in the eradication of infection in 26 of 27 patients, doxycycline produced a similar effect in 21 of 29 patients (p=0.017). Wilprafen therapy was better tolerated by the patients (p=0.033). It is concluded that wilprafen is a more efficient drug for the treatment of urogenital mycoplasma infection than traditional doxycycline therapy.
