Intra-abdominal testicular torsion in a cryptorchid dog

Intra-abdominal testicular torsion in a cryptorchid dog

Wiener Tierärztliche Monatsschrift 93(3): 58-61

ISSN/ISBN: 0043-535X

A 4.5-year-old, cryptorchid, male Greyhound was presented with abdominal pain at the emergency service of the University of Veterinary Medicine, Austria. Clinical symptoms of the dog were reluctance to walk, anorexia and lethargy for 2 days. The clinical examination revealed brick red mucous membranes with a prolonged capillary refill time. Cranial to the urinary bladder, a painful 10 x 10 cm structure was palpated. The pulse was of moderate quality at a rate of 120 beats/min. Haematology revealed leukocytosis with 19 070/ micro l (reference 6000-15 000/ micro l), monocytosis with 1277/ micro l (reference <500/ micro l) and segmented granulocytes with 16 095/ micro l (reference 3300-11 250). The liver and renal enzymes were within normal limits. Abdominal ultrasonography showed torsion and neoplasia of the left testicle. An emergency laparotomy was performed by ventral coeliotomy. Minimal serosanguineous fluid was suctioned, and the left testicle was located parasagittaly in mid-abdomen with a clockwise rotation of more than 360 degrees of the spermatic cord, 4 cm proximal to the testicle. The testicle was enlarged with haemorrhagic infiltration. The testicular artery, vein and ductus deferens were ligated separately with resorbable suture material. The right cryptorchid testicle was atrophied and removed in the same manner. After inspection of the abdomen, the abdominal wall was closed in a routine manner. The dog recovered uneventfully. A sertoli cell tumour was confirmed histopathologically. Differential diagnoses for acute abdominal pain included ileus, prostatitis, prostatic carcinoma, prostatic cyst, peritonitis, pancreatitis, urethral obstruction, acute pyelonephritis, splenic torsion, testicular torsion, testicular neoplasia and tumour of the bladder wall. Testicular torsion was commonly linked to neoplastic enlargement of the retained testicle.
