Bronchopulmonary and pleural aspergillosis

In 1963, at the age of 43 yr, the patient noted onset of dyspnoea and was found to have an abnormal chest X-ray. Histplogical examination of a scalene lymph node led to a presumptive diagnosis of sarcoidosis and prednisone was given briefly. During the next 6 yr he had repeated episodes of ' flu ' with cough and progressive dyspnoea. In 1969 a bronchopleural fistula and pyopneumothorax developed and Aspergillus fumigatus was found repeatedly in the sputum and pleural fluid.
