Comparison of long-term effects of saluretics and of anabolic steroids on renal functions

In a group of 20 elderly hypertensive and normotensive residents of a Home for the Aged, renal functions were studied before and after three to 6 mo. of therapy with 50 mg hydrochlorothiazide daily. These subjects had renal plasma flows (KPF) averaging 60% and glomerular filtration rates (GFR) averaging 70% of those of middle-aged persons. Blood pressure was reduced in all subjects, including the few normotensives who completed the period of treatment. At the end of treatment, CIn and CpAH were significantly decreased to means of 83.4% and 86.1% of the respective control vaues, while Tm PAH decreased[long dash]though not significantly -to 90.5%. A moderate hypopotassaemia developed in all subjects, but no significant correlation between it and the decrease in CIn. or CPAH was present. Tubular functional mass changes were quite unrelated to changes in serum potassium. Since the average reduction in mean blood pressure (-15.3%) equalled that of renal plasma flow, whereas total renal resistance was not consistently reduced (-7.8%), decrease in vascular resistance must have occurred in beds other than the renal. Diametrically opposite renal functional responses were obtained by administration of 150 mg methandrostenolone or nandrolone phenylpropionate over a 6-week period to 12 subjects, two of whom were simultaneously receiving hydrochlorothiazide. Significant increases in GFR and Tm PAH were observed; RPF and renal resistance increased and decreased respectively, but these changes were not significant. Anabolic treatment did not affect blood pressures or the serum levels of sodium and potassium. The possibility that anabolic steroids could be used in long-term saluretic therapy to forestall renal functional impairment might be worthy of clinical trial.
