Corticosteroid Choice In Allergy

Corticosteroid Choice In Allergy

Annals of Allergy 21: 454-465

For suppressive therapy during the acute allergic process 2 tablets 4 times daily of any of the major steroids are usually adequate. Triamcinolone is theoretically superior in asthma complicated by right heart failure, and dexa-methasone in the depressed patient. For maintenance therapy flu-prednisolone and paramethasone proved best, Dut a single daily dose of any suitable immediate-acting oral preparation is satisfactory. Prolonged oral absorption preparations were unnecessary. Combinations of steroids with sedatives, antihistamines or ataractics were advantageous. Of the injectable depot steroids methylprednisolone is the preparation of choice. For topical steroid therapy of the skin, flurandrenolone, dexamethasone and triamcinolone are preferred, with dexamethasone preferable also for topical treatment of the nose and eye. Steroid aerosols for asthma offer the advantage of decreased dosage. Dexamethasone aerosol proved satisfactory in a majority of cases.
