The diagnosis of intrauterine fetal death may be difficult. Estriol excretion drops suddenly after fetal death. The absolute values are of less importance than the sudden drop in serial estriol estimations. The reason is the reduction of the capillary action by which transport of blood from the intervillous space of the placenta is nearly brought to a halt. The flow of blood in the intervillous space of the placenta is dependent on the capillary action of the fetal vessels in the villi.
