Pulmonary clearance of radioactive iodinated serum albumin in man

A method is presented for estimating the uptake and clearance of albumin by the lung through the use of external scintillation counting. The 15 subjects studied fell into 3 groups: A, 1 normal subject and 8 patients without evidence of pulmonary edema or congestive heart failure; B, 4 patients with congestive heart failure and pulmonary edema; C, 2 patients with "pulmonary disease," 1 with uremic pneumonitis, and 1 with alveolar proteinosis. The disappearance from the blood and lung of intravenously administered I131-tagged human serum albumin (RISA) was measured. RISA blood clearances were similar in all groups but clearances from the lung after 24 hours in C were significantly slower than for A or B. B showed clearance rates intermediate between those of A and C. External scintillation detection of pulmonary RISA clearances is suggested as a simple technique to aid in the differential diagnosis of pulmonary disorders.
