A 33-year-old man with primary pulmonary hypertension underwent a single right lung transplantation in 1990 His immunosuppressive regimen consisted of azathioprine, ciclosporin and prednisone. Four months after transplantation, he was diagnosed with cytomegalovirus pneumonia and treated with intravenous ganciclovir. Six weeks later, he presented with productive cough and severe dyspnoea.
