Introduction: Disturbances of gastric motility in Parkinson's disease may be an important mechanism that determines variability in levodopa bioavailability. Aim: The aim of the present study was to evaluate the simple and non-invasive ABT and OBT of a liquid and of a solid test meal in patients with Parkinson's disease by using an isotope-selective non-dispersive infrared spectrometer (NDIRS). Material/methods: 35 patients with idiopathic Parkinson's disease (range 39-83 yrs., median UPDRS: 37,5)off medication affecting gastric motility and 20 healthy volunteers (range 48-71 yrs.) were given a solid test meal (250 kcal) containing 100 mg 13C-sodium-octanoate. 40 patients (range 41-79 yrs., median UPDRS: 45) and 20 healthy volunteers (range 41-88 yrs.) were investigated with a liquid test meal (350 ml Fresubin diabetes, 315 kcal)containing 100 mg 13C-sodium acetate. In each breath sample, which were obtained before substrate administration and in ten minute intervals over four (OBT) or three (ABT) hours test time 13CO2/12CO2-isotopic ratio was determined by NDIRS. The results were expressed as percentage and cumulative percentage doses of 13C recovered (PDR, cPDR) for each time interval and time of maximal PDR (tpeak (min)). Breath test gastric half emptying time was calculated of the OBT and ABT (t1/2b (min)). Statistics: u-test. Results: Parkinson patients had a significant delay in gastric emptying in OBT (tpeak : 117 +- 37 vs. 73 +- 19 min, t1/2b: 169 +- 42 min vs. 107 +- 9 min (p < 0.0001)) as well as ABT (tpeak: 84 +- 31 vs. 69 +- 21 min, t1/2b: 141 +- 62 min vs. 107 +- 19 min (p < 0.05)). Conclusions: Patients with idiopathic Parkinson's disease with different disease severity have significant disturbances in gastric emptying of a solid and a liquid test meal compared to healthy control subjects. This has important implications for treatment strategy in Parkinson's disease.
