Multi-patient mapping of language sites on 3-D brain models

The variability of essential language sites dictates that cortical stimulation mapping (CSM) be performed to plan surgical resections of tumors or epileptic foci in the cerebral cortex, so that language sites can be spared. These CSM data provide a unique opportunity to study language organization in individuals and across a population. We have reconstructed individual brains from pre-surgery MRI series (1.5 Tesla SIGNA, GE), and plotted the CSM sites on reconstructed 3D brain models, by visually matching the rendered surface and vascular patterns with a photograph of the exposed brain and CSM sites taken during surgery (Modayur et al., Neuroimage, 6:245-258, 1997). In some patients, functional MRIs during language tasks were also obtained, and plotted onto the reconstructed brains. To compare language organization across patient populations, we have begun plotting cortical sites essential for language from several patients onto an averaged brain model. We are utilizing a rigorous volume matching strategy across the entire brain (http://www.bic.mni.mcgill.ca/software/ mni_autoreg/node7.html) to produce linear transforms which allow accurate transfer of spatial locations from individual brains to the average brain. Multi-patient maps were generated which summarize essential language sites for various patient groupings. Comparisons are made between language sites of patients with right-brain dominant language and those with left-brain language dominance (determined by intracarotid amobarbital injection). Other groups compared are patients with low versus high verbal IQ's (VIQ's) and males with low VIQ's versus females with low VIQ's.
