Multicenter evaluation of the BD PhoenixTM System for detection of oxacillin resistant Staphylococcus aureus

Background: Identification of oxacillin resistant S. aureus (ORSA) continues to challenge rapid test systems. Methods: A multilaboratory evaluation was performed to compare the performance of the BD PhoenixTM Automated Microbiology System (BD Diagnostic Systems, Sparks, MD, USA) with the NCCLS Standard Broth Microdilution (SBM) containing Mueller-Hinton (MH) media and with the Oxacillin Screen Agar, BBL for the detection of ORSA. Discrepant results were resolved by repeating the tests in duplicate for Phoenix and SBM, and repeating once in OSA. The SBM system results in the first study and the Oxacillin Sceen Agar in the second study were accepted as definitive. Results: A total of 785 strains of S. aureus were tested in the Phoenix System and SBM System. The Phoenix System detected 306 (98.1%) of 312 ORSA, and 473 (100%) of 473 oxacillin susceptible strains; overall accuracy was 99.2%. A total of 800 strains of S. aureus were tested in the Phoenix System and the Oxacillin Screen Agar test. The Phoenix System detected 312 (98.4%) of 317 ORSA, and 482 (99.8%) of 483 oxacillin susceptible strains; overall accuracy was 99.3%. Conclusion: This demonstrates that the Phoenix System is comparable to the reference Standard Broth Microdilution and the Oxacillin Screen Agar method for detection of oxacillin resistance in Staphylococcus aureus.
