Multicenter external quality assessment for the molecular detection of human cytomegalovirus in the Belgian centers for molecular diagnostics

Background: Funded by the Belgian Health Insurance, a network of CMDs provide clinically relevant molecular diagnostic testing. A national external quality assessment was done to evaluate the CMDs performance in molecular detection of HCMV. Methods: Panels consisted of 24 coded samples composed of amniotic fluid (AF) or Hank's balanced salt solution (HBSS) buffer spiked with dilutions of AD169 and Towne virus stocks. Methodological information was obtained by means of a questionnaire. Results: Twenty-one data sets were received for 16 CMDs participating in the survey. The performance of each data set was measured by assigning a score: one point was given for every correct result. False positive, false negative and equivocal results were not scored. Sixty-six percent of participants obtained scores gtoreq22, including 42.8% data sets that achieved the maximum possible score (24). The lower score was 18 (75% of correct results) and was obtained by 2 laboratories. The majority of the labs were able to detect 20 pfu/ml in HBSS and 127 pfu/ml in AF. Sensitivity for 1.27 pfu/ml in HBSS and in AF decreased to 13/21 (62%) and 9/21 (42.8%) data sets. The combined used of Qiagen extraction method and real-time or nested PCR correlated with higher scores. Other methodological aspects such as primer sequences and sample volume could not be clearly associated with performance. Conclusion: The overall performance of the CMDs was very good, all participants achieving satisfactory or excellent scores. Results indicated that the DNA extraction method is an important factor that influences the performance of the assays. This multi-center quality assessment scheme contributed to the optimization and standardization of molecular diagnosis of HCMV infections in Belgium.
