Electro- and phonocardiograms in postinfarct defect of the interventricular septum

A detailed comparison of clinical data with ECG and PCG [phonocardiography] findings in patients with the postinfarction ventricular septal defect demonstrated distinct parallels. In patients with anterior myocardial infarction, there was anteroinferior localization of the ventricular septum, while in those with posterior myocardial infarction posterosuperior localization. In anterior infarctions, ECG changes were sharply pronounced and attested to the presence of the aneurysm of the left ventricle. That was a direct reflection of severe affection of the myocardium, leading to rupture of the ventricular septum. Systolic murmur on the ECG was registered in all patients and was band-shaped or indefinite in form. The maximal intensity of the murmur was recorded in the 4th intercostal space at the left margin of the sternum (2nd point) in anteroinferior ruptures and in the 3rd intercostal space at the left margin of the sternum (3rd point) in posterosuperior ruptures of the ventricular septum.
