A patient who would otherwise be at low risk for cerebrovascular accident had a right hemiparesis and aphasia. Evaluation revealed iron deficiency anemia secondary to menorrhagia and marked thrombocytosis, which responded to intramuscular and, later, oral iron replacement. Hemiparesis and aphasia resolved after a month.
