Evaluation of a rapid immunochromatographic assay for detection of Streptococcus pneumoniae antigen in urine samples

A multicentre study was conducted to evaluate a rapid immunochromatographic membrane test, the Now S. pneumoniae Urinary Antigen Test (Binax), in adults admitted to 3 French hospitals with clinical and radiological evidence of community-acquired pneumonia. From January to May 2000, ninety one cases of pneumonia were included in the study. The diagnosis of S. pneumoniae pneumonia was established in 28 of the 91 total patients by standard bacteriological techniques. The S. pneumoniae Now test was positive for 23 of these 28 patients, giving a sensitivity of 82%. Of the 63 patients who did not have pneumococcal pneumonia, 14 were positive in the S. pneumoniae Now test; crude specificity value of 77%. 13 positive results were observed in patients with no aetiological diagnosis. The specificity of the Now test was further studied using urine from healthy subjects. False-negative results with this test were observed in 5 patients with positive S. pneumoniae cultures. The positive predictive value of the Now test (ratio of 23 patients in whom both the Now test and conventional culture were positive over the total number of patients in whom the Now test was positive) was 62%. The negative predictive value (ratio of 49 patients in whom both the S. pneumoniae Now test and conventional culture were negative over the number of patients in whom the Now test was negative) was 90%. It is concluded that the Binax Now S. pneumoniae Urinary Antigen Test may be a useful additive diagnostic tool for community acquired pneumonia in adults.
