Summary
For diagnosing filariasis a combination of clinical examination and microscopic examination of 1-4 drops (20 cmm each) of blood are recommended. Among 291 positive but lightly infected persons, micro-filariae were found in 45% in the 1st drop of blood, in 62% by examining 2 drops, in 82% by examining 3, and in 100% by examining 4 drops. Of 5 schedules for prophylactic administration of diethylcarbamazine to eliminate microfilariae, the best was 3 mg/kg twice a day for 1 day each month. Diethylcarbamazine appears to have little if any effect on the adult nematodes. It causes the eosinophiles to increase due to release of foreign protein by the dead microfilariae, but this later subsides. The whole population of Tahiti is being treated prophylactically with diethylcarbamazine. Before treatment 39% carried microfilariae; 8% of the persons treated for 1 year, 6% of those treated for 2 years, and 2.5% of those treated for 4 years were positive. The mean annual numbers of lymphangitis attacks in the above persons were 0.82, 0.53, 0.26 and 0.13, respectively. One group of more than 50 patients was treated with diethylcarbamazine daily for 21 days and another similar group daily for 28 days. Chronic signs of the disease were not modified, but microfilariae reappeared in the blood following cessation of treatment less often than in persons treated daily for 7 days. Treatment of hydrocele by simple drainage was permanently effective in 25% of 125 persons, but treatment by drainage after injection of sclerotizing substances was permanently effective in all of 22. Treatment of elephantiasis of the legs by application of "Unna" type paste and bandaging was ameliorative in more than 200 persons.
